Stoddard County Public Health Center can print certified copies of birth and death certificates from the state of Missouri only.

Certified copies of Birth Certificates can be printed at our office while you wait. Certificates are available for any birth occurring in the State of Missouri after 1920. Birth Certificates are $15.00 each.

Certified copies of Death Certificates can be printed for any person whose date of death is after 1980. Death Certificates are $14.00 for the first one, and $11.00 for each additional copy if purchased at the same time.

The Health Department can only accept cash or money orders.

To request a death certificate, print the following form, complete, and bring or mail to the Health Department along with the money order per certificate fee (must be notarized).

Applicants must show identification when requesting certified copies of a vital record at the state health department. Mail-in requests must be notarized by an acceptable notary public.

You need:

• Full name
• Date of birth or death
• Hospital where birth or death occurred
• Your relationship to the individual named on the record
• The purpose for the copies
• For birth records, also include Father's name and Mother's maiden name
